Child Information Placement Referral and Transition Planning (eLearning)

You will learn how to write a strength-based, trauma-informed, culturally responsive and LGBTQIA+ affirming Child Information and Placement Referral (CIPR, CHIPR or 15-300). Understanding how the Child Information and Placement Referral (15-300) impacts child and youth placements is a primary focus. 

You will be introduced to policy requirements and practice expectations, including using child and youth voices to tell their own stories. You will gain an understanding of the importance of working with caregivers to plan for placement transitions that support child and youth wellbeing. You will also gain insight into how to ensure the Child Information and Placement Referral (15-300) includes the information needed to safely meet the needs of the children and youth transitioning into homes.

Knowledge
•    Recognize what information to include in a well written Child Information and Placement Referral and the impact this can have on placement stability.
•    Know how and when to update the Child Information and Placement Referral, who is responsible for acknowledging receipt, and where to upload in Famlink.
Skills
•    Be prepared to gather information from multiple sources to communicate support needs in a strength based and trauma informed way. 
•    Be capable of accurately completing a Child Information and Placement Referral that would support the caregiver in safely meeting the child’s needs.
